What do I need to do for a religious exemption?
A Religious Exemption Form must be completed and submitted to the school. This form is available on the Murfreesboro City Schools website or from your child’s school nurse. Once completed, please return it to the school nurse.

How do I know what immunizations are required for my child to attend MCS?
Required immunizations are based on Tennessee state guidelines. Please visit the Immunization Information section on this page for details, or contact your child’s school nurse for assistance.

Where can my child get a physical exam?
Physical exams may be completed by:

  • Your child’s pediatrician or primary care provider
  • The local health department
  • Urgent care or walk-in clinics

My child needs medication or a procedure at school. What do I need to do?

A Medication/Procedure Authorization Form is required for all medications or procedures administered at school. A new form must be completed and submitted at the beginning of each school year. Forms from previous school years are not valid and will not be accepted. Updated forms are also required anytime there is a change in your child’s medication or physician orders.

  • All medications/procedures require:

    • Parent/guardian signature
    • Written orders from a licensed healthcare provider

Forms are available on the MCS website or from your child’s school nurse.

Can my child bring medication to school?
No. All medications must be delivered to the school clinic or front office by a parent/guardian or an approved adult designee.

Exception: Students may carry and self-administer emergency medications (such as inhalers, EpiPens, or diabetic supplies) when proper documentation is on file.

When should my child stay home due to illness?
Please refer to the Illness Guidelines section on this page for information on when to keep your child home and when it is safe to return to school. If you have questions, contact your child’s school nurse.
