Proof of Address:
Electric, Gas, Water, Cable, or Bellsouth current bill
with parent's name and address clearly stated

(Please note that cell phone bills are not acceptable.)

Student's Social Security Number

Emergency Contact Information

Students new to Tennessee will also need the following information
for enrollment:
Birth Certificate or other legal proof of date of birth
TN Health Card (available at Rutherford Co. Health Dept.)
Name, Address, & Phone Number of last school attended

Students transferring from another TN school will need the above
information with the exception of the health card. It will be sent
with the student's records from the previous school.
