Proof of Address:
Electric, Gas, Water, Cable, or Bellsouth current bill
with parent's name and address clearly stated
(Please note that cell phone bills are not acceptable.)

Student's Social Security Number

**Immunization Record**
(This is a new requirement for the 2008-2009 school year)
Students transferring from other TN schools will need to bring their
TN Health Card/Shot Record. This MUST be presented on the day of registration.
All other student records will arrive once requested.

Emergency Contact Information

Students new to Tennessee will also need the following information
for enrollment:
Birth Certificate or other legal proof of date of birth
TN Health Card (available at Rutherford Co. Health Dept.)
Name, Address, & Phone Number of last school attended
